Accountability

The Local Health System Integration Act, 2006 requires that the Toronto Central Local Health Integration Network (LHIN) and Health Service Providers (HSPs), like SPRINT Senior Care, enter into a service accountability agreement. The service accountability agreement enables the LHIN to provide funding to the HSP for the provision of services. It also supports a collaborative relationship between the LHIN and the HSP to improve the health of Ontarians through better access to high-quality health services, to co-ordinate health care in local health systems, and to manage the health system at the local level effectively and efficiently.

Privacy

SPRINT Senior Care is committed to protecting the privacy and confidentiality of the personal information of clients, volunteers, donors, staff, and other stakeholders.

What is PHIPA?

The Personal Health Information Protection Act (PHIPA) 2004 governs how your personal health information may be collected, used, and disclosed. It also allows you, the client, control of how and with whom your information can be shared with your consent.

What information might SPRINT Senior Care collect?

SPRINT Senior Care collects your personal health information, which is any health or health care information that could identify you alone or in combination with other information. Depending on the services you request, this may include:

  •     your name, address and phone number;
  •     your date of birth;
  •     your Ontario Health Card number;
  •     your health history;
  •     the health history of you and your family;
  •     allergies and medications;
  •     the reasons you need service from SPRINT.

Why does SPRINT Senior Care collect your personal health information?

SPRINT Senior Care collects your personal health information to assess your needs and to provide health and social services relevant to your needs, as well as to share your information with others involved in planning and providing your care.

When your personal health information is no longer required, your personal file will be closed.

What are your rights regarding the sharing of your personal health information?

It is your right to decide who has access to your personal health information. All exceptions to the sharing of your information will be recorded on the Privacy Consent Form.

How can you access your health records at SPRINT Senior Care?

SPRINT Senior Care's Privacy Officer ensures that your personal health information is properly treated and maintained. You have the right to review your personal health information with the Privacy Officer at any time. You also have the right to request a correction or amendment to your personal health information at any time.

You will need to identify yourself to the Privacy Officer in order to request changes to your Assessment. You may need to provide your name, address, and telephone number.
